This Pre-Application for Housing Assistance requires Adobe Acrobat Reader to open.

If you do not have Adobe Acrobat Reader, go to: http://www.adobe.com/products/acrobat/readermain.html and download the free program.

To download the pre-application to your computer, right click on the link below and select "Save target as..." from the menu.  Once you have saved the file to your computer, you can open it.  You can also left click on the link to open the pre-application without saving it to your computer.

Once you have opened the pre-application with Adobe Acrobat Reader, you have two choices:

1. You can fill the application out on your computer by clicking on the appropriate boxes with your mouse to enter checkmarks and to type the required information.  Once you have filled out the application by using your mouse and keyboard, review your selections and check for completion; print the application.  Mail the application to one of the addresses on the application or FAX it to one of the FAX numbers on the application.
2. You can print the application and fill it out by hand.  Mail to one of the addresses on the application or FAX to one of the FAX numbers on the application.
