You may have one dog or cat weighing no more than 30 pounds with an approved pet application and a $150 deposit. Download the Pet Policy and Rules document for more information. Download and fill out the Pet Application, and turn it into your Property Manager. Remember, you cannot bring a dog or cat into your unit until you have approval.
Download the Verification of Need for Wheelchair Accessible Unit form and take it to your doctor. Once the form is completed, have your doctor return it to HACSA by fax, or you may bring it to your Property Manager yourself. Wheelchair Accessible units rarely come available, but current residents are given first preference when filling them.

Please download the Verification of Disability Status for Reasonable Accommodation and the Request for Reasonable Accommodation forms. Take these two forms to your doctor, psychiatrist, therapist or any other qualified individual who can attest to your disability-related need for a companion animal. If you have any questions about the Reasonable Accommodation process, contact your Property Manager. After you have turned in the first two forms and your Reasonable Accommodation request has been approved, you will need to download and submit the Companion Service Animal Registration form for approval, before bringing the animal into your unit. See the Assistance/Companion/Service Animal Guidelines for more information. There is no pet deposit for an approved companion or service animal.
Please download the Live-in Aide Medical Need form, the Live-in Aide Request and Application form, the Live-in Aide Criminal History form, and the Designation of a Family Member form. Take the Medical Need form to your doctor and have your doctor return it to your Property Manager. Complete the Live-in-Aide Request & Application form and the Criminal History form with your Live-in-Aide. You and your Live-in-Aide will also need to complete the Designation of Family Member form. Return all of these forms to your Property Manager. Once your application to add your Live-in-Aide has been approved, you will be placed on a transfer list to move to an appropriately sized unit for your new household composition.
